Individuals or Healthcare organizations may send a referral for Home Help services by phone at (616)730-3805 or Online Submission Portal. Referral information must be entered below for consideration. The referral source does not have to be the individual in need of the services.
One of our skilled and experienced Health Care Coordinators will follow up with the referred client. The client will be scheduled for a teleconference. to confirm eligibility for program. Upon successful completion of eligibility, the client will be scheduled for an in-person Health Assessment.
We offer a range of in-home health care services, including medication management, chronic disease management, and more. Our services are tailored to meet the unique needs of each individual. Following the Health Care Assessment the client will receive an Individualized Care Plan (ICP). We work closely with families and Health Care Providers to ensure that our clients receive the best possible personalized care.
